If you’ve been feeling off for months — exhausted no matter how much you sleep, gaining weight without explanation, snapping at people you love for no reason — I want to start by saying something I say to almost every new patient I see: you’re not imagining it. Sign #1: Fatigue That Sleep Simply Does Not Fix

When I review bloodwork with patients who describe this kind of fatigue, I’m often looking at the full thyroid picture — not just TSH, which is the only test most primary care offices run. Free T3, free T4, reverse T3, and thyroid antibodies can all tell a very different story. I’m also looking at cortisol patterns. When your adrenal glands are producing cortisol at the wrong times — too high at night, too low in the morning — your sleep cycle never fully restores you. This isn’t a willpower issue. What’s typically happening is a combination of estrogen dominance, declining progesterone, and early insulin resistance — a trio that tells your body to store fat centrally and hold on to it. Many of the women I see have already been offered antidepressants at this point — and while that approach is sometimes appropriate, it’s rarely the first question I’d ask. My first question is always: have your hormones been properly evaluated?

Progesterone is your body’s natural calming agent. It works on the same receptors as anti-anxiety medications. When progesterone drops — as it does in perimenopause, or under chronic stress — the result often looks exactly like an anxiety or mood disorder. Estrogen fluctuations destabilize serotonin. Thyroid dysfunction clouds emotional regulation. These are treatable root causes, not character flaws. You’re not losing your mind. Your hormones may simply be out of balance.

Sign #4: Hair Thinning, Skin Changes, or Disappearing Libido

These symptoms tend to arrive quietly and get dismissed as aging. Hair thinning at the temples or the top of the crown. Skin that’s drier than it used to be, or suddenly breaking out. A libido that has gone completely silent — not just low, but absent. I want you to know: these are not vanity concerns. They are your body communicating a real hormonal shift.
