  • Medroxyprogesterone acetate (MPA) — a progestin, not progesterone. It binds progesterone receptors but triggers very different downstream effects.

This is the combination used in the original Women’s Health Initiative (WHI) study in 2002 — the one that scared a generation of women and their doctors away from hormone therapy. The headlines said “hormones cause breast cancer.” But what the study actually tested was CEE + MPA. Not estradiol. Not micronized progesterone.

And even those WHI numbers have been re-examined. As Dr. Avrum Bluming puts it, “the risk of breast cancer development on the graphs that they publish for the combination are identical to the risk for estrogen alone.” Two decades of fear built on a reading of the data that doesn’t hold up.

Here’s where I want to be honest with you. “Bioidentical” does not automatically mean “compounded.” That’s a common misunderstanding.

There are two categories of bioidentical hormones I prescribe:

  • FDA-approved bioidenticals. Estradiol patches, estradiol gels, and oral micronized progesterone (the capsule form). These are covered by most insurance plans.
  • Custom compounded preparations. Made by a compounding pharmacy when a patient needs a dose or delivery method that isn’t commercially available — for example, a specific testosterone cream concentration or a vaginal progesterone for a patient who can’t tolerate oral.

Both can be appropriate. Neither is automatically better. I choose based on what your bloodwork, symptoms, and tolerance tell me — not based on what sounds more “natural.”
